Community Outreach Funds Program Protocol
Notice of Funding Opportunity Distribution Protocol:
To notify the community of the funding opportunity available, the Community Outreach Funds Program one-pager will be posted to Fitness for Life Around Grant County’s social media platforms and shared by active members through email distribution to eligible agencies and/or organizations (i.e., Chamber of Commerce, Grant County Resource Team, etc.). The one-pager will include a link and QR Code that directs applicants to Fitness for Life Around Grant County’s website to download the Request for Funding Form in Word Document format with instructions to submit the form via email (see Application Submission Protocol section).
Application Submission Protocol:
To be considered for funding, a representative from eligible organizations must have participated in at least two Fitness For Life Around Grant County events or meetings within the last twelve months (prior to submission). All applications must be submitted and approved prior to the event/project occurring. All applications must be submitted via email to GetFitGrantCounty@gmail.com by 5:00 pm on the application deadline date. Applications received after the deadline will not be accepted. All applications will be reviewed by Fitness For Life Around Grant County’s Executive Board for scoring and recommendation to the coalition for approval (see Application Review and Approval Protocol). Applications must indicate how their project or event will advance at least two healthy lifestyle behaviors, such as healthy eating, physical activity/active living, mental health or tobacco-free environments. Other application eligibility requirements include: Events/projects selected for funding will be asked to have youth participants and/or volunteers, make available education materials provided by the coalition, and acknowledge Fitness for Life Around Grant County on all media and signage.
Application Review and Approval Protocol:
All applications submitted by the application deadline will be reviewed and scored by no less than three of the four Officers on the Executive Board. After individual scoring by Officers, the Executive Board will review scores as a committee to determine which applications will be recommended for coalition approval. Coalition approval will occur during the regular monthly meeting, and notification of the award will occur the following week.
Post Award Protocol:
Awarded application projects/events must be completed within 1 year, and the applicant organization must submit a post-award evaluation survey to be reviewed by Fitness for Life Around Grant County’s Executive Board.
