Community Outreach Funds Program

 

FFLAG is committed to supporting community and school events or projects focusing on healthy lifestyles. 

What it is:

The Community Outreach Funds Program is a community-wide initiative to work with and support community organizations and schools at the local level to advance healthy eating, active living/physical activity, and tobacco-free initiatives through policy, systems, and environmental change projects and events that will improve the health outcomes of Grant County. This initiative will provide funding and technical assistance to assist community and school events or projects focusing on healthy lifestyles. 

While not limited to the following topics, example proposals could be related to:

· Health Equity

· Public Health Transformation

· Healthy and Safe Communities

· Quality Improvement Projects

FFLAG will award funding for up to 3 projects or events with funding not to exceed $5,000 per project.

 

Objectives for funding include:

· Implement a project or event to advance healthy lifestyles through healthy eating, active living, and tobacco-free initiatives.

· Engage youth within the project or event.

· Engage and communicate with partner organizations and local stakeholders to support the project or event.

· Acknowledge FFLAG on all media and signage.

· Submit a final report (a post-evaluation survey will be provided for reporting requirements).

Criteria for Eligibility: 

· Application must be submitted and approved prior to the project or event.

· Funding will only be given to projects and events located in Grant County.

· An applicant organization representative must have participated in at least two Fitness For Life Around Grant County events or meetings within the past twelve months.

· Events or projects selected for sponsorship must be completed within 1-year of the award.

· Funding cannot be used for salaries.

· Only one application will be considered per organization during this cycle.

· Maximum request amount per application is $5,000.

· In-kind funds are not required but do show community involvement and possible sustainability.
